SUGGESTED PLAN
FOR UPGRADING DEVELOPMENT OF THE MEDICAL SERVICE DIRECTORATE UNITS AND HOSPITALS
MEDICAL SERVICES DIRECTORATE
GENERAL INTRODUCTION.
The health sector improvement is one of the most crucial and humanitarian priorities of the Palestinian people and PNA. Therefore, the Medical Services Directorate was established in 1994 as an integral part of the Palestinian health care delivery system; it works in close coordination and cooperation with the Ministry of health and the superior health council contributing in developing the health sector with the other Palestinian health organizations (governmental and non-governmental).
The MSD’s mission is to provide comprehensive health services (preventive, curative, primitive and rehabilitative services) to all police and national security forces members with their families and the neighboring civilian.
In other words, the MSD’s role is humanitarian and essential: Providing medical care.
The total staff of MSD hospitals, health care units are 1258 persons (986 Male and 272 Female) by the end of year 2004 are as follows, ( General practitioners 165, specialists 120, pharmacists 45, dentist 55, vetrinitatries 10, engineering's 14, Nurses 198 in addition to the students in sanable nursing school 85, Administrative staff 476 and Others employees 90 ).
Financially, The main budget of MSD essentially from the monthly premium for health insurance of all members of police and national security forces according to the MSD insurance program.
According to MSD system confirmed by the Police and National Security leader .
The head quarter of MSD located in Ansar / Gaza, with a net work composed from 7 medical company ( 4 in the southern governorate & 3 in the northern governorate), composed from:17 medical units, 16 medical points, in addition to 12 comprehensive health care units and 3 general hospitals (Al-Karama hospital and Balsam hospital in southern governorate & Palestine hospital / Harmala in the northern governorate ), distributed all over the PNA governorates.
The MSD started to provide health services through 3 levels:
I- Curative medical services, which includes:
a)- Medical services units
b)-Comprehensive medical services.
c)-Advanced medical services.
II- Pharmacies services.
III- Dentist services
CURATIVE MEDICAL SERVICES:
a)- Medical services units
These units provide medical services to all police and national security forces members in their Battalion and according to their administrative and localities distribution, including first aid emergencies, general medical examination, laboratory analysis and medicaments.
b)-Comprehensive medical services.
These are the medical services provided to all police and national security forces members with their families and the neighboring civilian, outside the Medical services units, including the primary &secondary health care units.
*The primary health care units.
Includes the general and specialized medical examination in addition to the laboratory analysis,
x-rays and medicaments. There are 12 primary health care units distributed in all governorates throughout
*The secondary health care units (General hospitals).
Includes surgical ward, internal medicine ward and gynecology &obstetric ward, in addition to the x-ray &ultrasound, laboratory analysis and pharmacies services.
There are 3 hospitals, which are:
1-Al-Karama hospital-Abasan.
Located in Abasan (south-east of
functioning since 1995 (36 beds for surgical department, 6 beds for ICU, 22 beds for internal medicine including 4 CCU beds, 12 beds for gynecology&obstetrics).
Statistically, there are 310.158 patients visits the hospital until the end of year 2004, there are 7916 surgical operations were done (792 surgical operations every year).
2- Balsam hospital- Al-Nada district-
Located in Biet-lahia in the north of Gaza, functioning actually with 36 surgical beds in addition to 4 emergency beds, 8 gynecology&obstetric beds and 4 ICU beds, the different out-patients clinics and paramedical facilities.
functioning partially since June 1st 2002. 12 beds, ( 8 beds for surgical department, 4 beds for gynecology&obstetrics),
Statistically, there are 157500 patients visits the hospital by the end of year 2004, there are 1137 surgical operations were done.
The internal medicine ward not functioning ( accomplishments of construction and medical equipments are needed).
The first phase of this project was supported by (Save The Children) for construction of underground, first floor (concrete works only),
The second phase was supported by (Emirate Red Crescent Society) for construction of the second floor. This hospital will serve more than 190,000 population distributed on Bedween village, Ezbet Biet Hanon, El-Awda towers, Jabalia refugee’s camp, Biet Lahia and Tall El-Zaeter.
3- Palestine hospital – Harmala.
Located in Biet-lehem / West Bank this hospital the construction was completed, and will functioning after completing the medical and non-medical equipments needed.
Palestine hospital 68 beds distributed as follows:
23 surgical beds, 29 internal medicine beds, 4 emergency beds, 4 ICU beds, 8 Gynecology & Obstetrics beds..
c)-Advanced medical services
These are the medical services to help the insured sick people to be treated abroad in a highly advanced centers if they are in need and not present in the MSD hospitals.
The transferring process is controlled by medical committee and supervised by the treatment abroad department in MSD.
Different cases referred by MSD to:
1- Advanced center outside the Palestinian National Authorities territories, (To Egypt, Jordan).according to special contracts.
2- Medical centers inside PNA territories because lack of medical facilities at MSD.
3- Palestinian National Authority hospitals
FIRST AID AND EMERGENCY DAPARTMENT
Emergency department was established in MSD in 1994 , it began with one ambulance, doctor, and emergency officer. This department has an important role in rescue of injured people.
This department has 20 Ambulances (12 in Gaza Strip and 8 in West Bank).
At Gaza Strip there are 12 ambulances, 22 Ambulance drivers, 40 trained emergency officer, 4 doctors and 5 qualified nurses.
Emergency department responsibilities.
1-Provide medical emergency and first aid services to injured people.
2-Provide medical emergency care to sick people and transport them to different hospitals according to diagnosis.
3-Provide emergency medical services to our people in our disasters(Establish mobile emergency hospitals in that disasters with coordination and cooperation ( WHO and PRCS).
4-Training courses (first aid) to different agencies, schools, universities, and police forces).
5-Continuous education and training courses of first aid technique for MSD employees.
6-First aid education and training to students in summary camps.
7-Medical supervision on training courses of police forces.
8-Sharing in different national and popular activities.
THUS, there are 138Training courses concerning the emergency first aides arranged and organized by this division for different sectors (governmental, popular, establishments and associations).
The total numbers of participants who attended these courses from 1996 to 2004 are 7962 persons.
The mean for these training courses is15.3 course every year, and the mean for participants is 844.6 every year.
PREVENTIVE MEDICINE DEPARTMENT.
a) Environmental health.
Ensure good and suitable healthy conditions related to the housing, equipment’s of police and security force, and the surroundings through out:
1-Regular health inspection of all police and security forces units.
2-Eradication of insects and rodents.
b) Health promotion and education.
Aiming at reducing the mortality and morbidity among individuals by modifying the health related behavior and life style through different health programs.
c) Establishment of a system concerning the tables of narcotics and controlling the entry of such
drugs and goods until 01/12/1999.
d) Food and water safety.
Ensure the quality and safety of food products and services.
Reduce the contamination incidents for locally produced and imported food.
e) Infectious disease control.
Through prevention, detection, and treatment diseases.
f) Veterinary medical units.
Ensure food, safety in slaughterhouses and distributed centers.
The most important things which was achieved by this department are:
1- Ensure the quality and safety of food products and narcotics and controlling the entry of such drugs, in direct cooperation with ministry of health, ministry of supplementation , and the national foundation for criteria measurement.
2- Ensure the table of narcotics and the process of entry, registration and inspections.
PHARMACEUTICAL SERVICES.
This department has the following roles:
a) Distribution of medicine and medical supplies to medical units of MSD.
b) Quality control monitoring of drugs effects in cooperation of the scientific committee and its research and studies department.
c) On-going assessment of the needs and priorities.
d) Assurance of medications to all chronic diseases patients enrolled in the MSD insurance
programs.
The mean annually payment for the drugs is 1.712.357 $
HEALTH INSURANCE SYSTEM.
The main essential role of health insurance program offered by MSD is to help the insured members of police, national security forces and their families to be consulted, operated, and receiving the medications to be recovered and cured, and also to be referred abroad in a highly advanced centers to do further investigations or a special management not exists in PNA territories.
Beneficiaries.
Health insurance enrolls all members of police and national security forces with their families (223,548) persons enrolled until the end of year 2004. As shown in the following table.

MEDICAL SCREENING COMMITTEE.
1- The main medical committee.
2- Medical committee for new enrollees staff.
These committees which are approved in the north and south governorats, offers complete physical and psychological examinations to new enrollees staff and injured patients to be treated abroad if needed.
HEALTH CARE SERVICES TO PRISONERS.
MSD is responsible for health services delivery needed for all prisoners, including routine physical and clinical examination, ensure medicaments, psychological and emergency care.
TREATMENT ABROAD DEPARTMENT (OUT-SIDE AND IN-SIDE THE PNA TERRITORIES).
THE MAIN ROLE.
The main role of this department is to help the insured sick member to be treated and managed abroad in a highly advanced centers if they are in need to do further investigations or a special operations such as an open heart surgery, radiotherapy, chemotherapy.
The total numbers of patients transferred abroad for the above-mentioned purposes from 1994 to the end of year 2004 are 11398.
The mean annually transferred patients abroad is 1266
At 2004 there are 3379 patients were referred out-side and in-side PNA territories.1303 patients were referred to an advanced center Abroad in which they needs a special care and management, (the total estimated costs are 1.268.968,32 NIS).
In addition, 2076 patients were referred to another hospitals in-side PNA for a special care and management, (the total estimated costs are 922854 NIS).
Thus, the total estimated costs for all referral cases (inside and outside PNA territories are 2.191,822.32 NIS.
MEDICAL SERVICES DIRECTORATE LOSSES DURIN INTIFADA
Since the beginning of intifada, the MSD re-establish an emergency new strategic planning, aiming to provides an emergency health care to police, national security forces and their families.
The MSD leaders decided the following :
1- Redistribution of all emergency medical points, and surgical field hospitals, according to the geographical events, thus 47 emergency medical points and 15 surgical field hospitals were established.
2- Redistribution of the MSD human power, to be easily rejoins the newly working place at any time of all events.
3- Redistribution of emergency medical supplies and drugs to all MSD emergency centers.
MSD losses as a result of Israeli aggression, heavy shelling and bombardment since the beginning of intifada at 28/9/2000 until the end of year 2003 are as follow:
1** The human losses
The losses concerning the MSD staff are:
a) 2 Martyrs.
- Dr.Wael khwaiter at 10/4/2002 as a result of al-sudania medical unit heavy bombardments.
-Rescuer. Said youssef shalayel at 8/3/2002 as a result of direct bombardment to the ambulance at the moment when he was saving the life of an injured people in al-sudania district.
b) 6 injured.

UPGRADING DEVEOPMENT OF THE MEDICAL SERVICES DIRECTORATE UNITS AND HOSPITALS
The MSD upgrading depends mainly upon, health care units establishment instead of the previously medical units destroyed as a result of Israeli aggression, heavy shelling and bombardment , in addition to re- equipments with medical and non medical materials and, also accomplishment the hospital construction, equipments of already existing hospitals and medical units and supporting the human power needed to star functioning well.
